What To Expect If You’re New:

Welcome to our office! We’re glad you’ve chosen us and look forward to meeting you at your first appointment. Here are a few steps we recommend you take before you arrive to better prepare for your visit.

1. Print and fill out the following forms as completely as possible and bring them with you to your appointment. You will need all of the following:

• • Registration & Health History Form
  • Oral Health Risk Factors Questionnaire
  • Financial Policy Agreement
  • Patient Consent Form (HIPAA)
  • Consent of Disclosure Form (HIPAA)
  • Signature On File
  • Amend Patient Records
    
    

2. Read through our frequently asked questions to get the answers to some common inquiries made by clients.

3. Find your insurance card and bring it with you to your appointment. If you have no insurance, please bring an alternate payment method, such as cash, check, or a credit card (Visa or MasterCard only), since payment is requested in full on the day that services are rendered.
